It's not hop-on hop-off; you stay on for the whole circuit. One thing to know: Stop 3 at the Colosseum is temporarily suspended, so that stop isn't available right now, though the bus still passes the other seven. On Sundays and certain holidays, traffic restrictions in Rome can affect which stops are running, so the exact route might shift depending on when you go. The bus is wheelchair accessible on the lower level, though electric wheelchairs can't be accommodated, and the easiest boarding point for accessibility is Via Marsala at Termini Station.
