You keep climbing. The final stretch is narrow, the air gets thinner in a way that's more psychological than real, and then you're on the terrace at 136 meters. Rome spreads out below you: the Colosseum to one side, the Pantheon, Castel Sant'Angelo, the Tiber winding through the city. St. Peter's Square is a perfect oval from up here. You can see the Vatican gardens. On a clear day, you can see all the way out past the city limits.
